Rash on back, neck and near face?

my brother has a rash that covers the entirety of his back, neck, and near the hair line of his face. a couple years ago it was very itchy red and scabby. we went to a couple doctors but all they could do is give him harsh creams that made it worse. additionally, one doc claimed he might have scabies but when we went to another doc he completely dismissed that diagnosis. there have been some flare ups but things have relatively calmed down. but his neck, scalp and face is still affected by the rash. he still has massive scars on his back from the rash ..and it has turned in to a grayish/dark brownish color. he unconsciously scratches himself in his sleep and in the morning he has bloody scabs. we don't know what to do. we stopped using fabric softener and reduced the amount of detergent we use but it still flares up. if anyone has experienced something or knows what kind of skin disorder he has I would really appreciate the help.
